Licensed Mental Health Counselor (LMHC) - Orange County
As a member of a multidisciplinary team, provides clinical consultations with medical providers. Additionally provides brief and crisis interventions to primary care patients as needed. This position will include providing risk reduction, basic HIV education and medication/medical adherence through the Ryan White program. 
Essential Functions:
Provides brief intervention and crisis intervention as needed
Conducts initial patient assessments
Conducts psychosocial assessments
Develops treatment plans
Writes and maintains progress notes for all encounters
Performs case management duties as necessary
Participates in case staffing and administrative meetings
Acts as an advocate on behalf of the patient.
Provides emotional and practical support to patients, by engaging in problem solving.
Supports clients in practicing healthy behaviors.
Engages clients who have fallen out of care or have not entered care.
Follows-up with patients who miss appointments.
Helps remove barriers to attending medical appointments

 

Counselor/Therapist - Master's degree in social work, psychology, mental health counseling, or related field from an accredited University/College

Orange / Brevard and Seminole Counties - Full and Part-Time positions available

Candidates must demonstrate experience providing therapy or counseling in a behavioral health setting.

· Proficiency in conducting assessments, diagnosing mental health and substance use disorders, and developing treatment plans.

· Strong understanding of evidence-based therapeutic modalities and interventions.

· Excellent communication and interpersonal skills to engage with diverse populations.

· Ability to work collaboratively as part of a multidisciplinary team.

Registered Nurse (RN) - Current State of Florida RN license or valid eNLC multistate RN license

Orange and Brevard Counties - Full Time

  • Identifying and assessing client psychosocial, cultural, ethnic and religious needs.

  • Administering client medications and providing knowledge of medications and side effects.

  • Documenting client treatment details.

  • Assisting physicians with transcriptions and implementation of orders.

  • Maintaining compliance by completing necessary training.
